To facilitate a smooth hospital admission process, please complete the online admission form for the hospital where you are having your procedure.
If you are having surgery at Kings Street Private Hospital, Dr McGrath’s rooms will arrange the booking for your surgery or procedure and hospital stay, and forward your Request for Admission and Consent form to King Street Private Hospital’s Admissions Department.
